An application for compensation for personal injuries and death under the Accident Compensation Act 2017 must be made by completing an application form. This form can also be collected from any LTA office in Fiji. The form with all required documents can be posted or hand delivered  to 'The CEO, ACCF, LTA Head Office, Lot 1 Daniva Road, Valelevu, Nasinu and you can also submit the applcation at any LTA office in Fiji. A copy of  the application form with supporting documents can be emailed to info@accf.com.fj. Hard copies must also be submitted.


Requirements
Every application for compensation must include the following:

         Personal Injuries
•        Certified true copy of Photo ID e.g. Joint Card & Voter Card (Applicant & Injured Person)
•        Original Police report
•        Original Medical report
•        Medical Health Card
•        Certified true copy of Birth Certificate (Applicant & Injured Person)
•        Certified true copy of Bank Statement (Applicant)
•        Tax Identification Number(TIN)
•        Any other information required by ACCF

        Death
•        Certified true copy of Photo ID e.g. Joint Card & Voter Card (Applicant & Deceased Person)
•        Original Police report
•        Certified true copy of Birth Certificate (Applicant & Injured Person)
•        Certified true copy of Bank Statement (Applicant)
•        Tax Identification Number(TIN)
•       Certified true copy of Certificate of Death
•       Letters of Administration or Probate if deceased is 18 years or above
•        Any other information required by ACCF
